ConnectionsPlus Offering ‘Five Wishes’ Booklet
ConnectionsPlus Healthcare + Hospice is observing National Health Decision Day (NHDD) on Wednesday, April 16, with an open house in which local residents can pick up a copy of the “Five Wishes” booklet.
The annual initiative is dedicated to encouraging individuals to express their health care wishes and make advance care plans. Founded in 2008, NHDD aims to inspire, educate, and empower people to have conversations about their end-of-life care preferences.
Health care decisions can be challenging, especially during critical moments when individuals may be unable to communicate their wishes, said a spokesperson. NHDD emphasizes the importance of advanced care planning, ensuring that individual’s health care preferences are known and respected.
The “Five Wishes” booklet covers important topics such as medical treatment preferences, comfort measures, and personal values, making it an invaluable tool for starting these crucial conversations, said the spokesperson.
