ConnectionsPlus Healthcare + Hospice Receives Honor

ConnectionsPlus Healthcare + Hospice (formerly Rockbridge Area Hospice and Mountain Regional Hospice) has been named a 2023Hospice CAHPS Honors Award recipient by HEALTHCAREfirst, a leading provider of advanced analytics for hospice organizations. This prestigious annual review recognizes agencies that continuously provide a positive patient experience and high-quality care as measured by the patient and caregiver experience. It acknowledges the highest performing agencies by analyzing the performance of the Hospice Consumer Assessment of Healthcare Providers and Systems survey satisfaction measures.

ConnectionsPlus continues to maintain a 5-Star rating by the Centers for Medicare and Medicaid Services, which put them in the top 10 percent of hospices nationwide. The rating is part of CMS’ Hospice Compare program that gathers data to assist families in choosing a hospice program for their loved ones. The data is compiled from family and caregiver surveys measuring care quality. The program measures eight key areas: communication with family, getting timely help, treating patients with respect, emotional and spiritual support, help for pain and symptoms, training family to care of the patient, willingness to recommend, and rating of the Hospice provider.

ConnectionsPlus has continued to invest the time and resources to serve all the people who need them.
